Entering Patient Information

Learn how to fill in patient details for your appointment.


The Patient Information Form

After selecting your date and time, you'll need to enter patient details:

Required Information

For each patient, you must provide:

Personal Details:

  • First Name - Legal first name
  • Last Name - Legal last name
  • Date of Birth - Select Month, Day, and Year
  • Gender - Male, Female, Non-binary, or Other

Contact Information:

  • Email Address - For confirmation and reminders
  • Phone Number - 10 digits, Canadian format (optional but recommended)

Booking for Multiple People

If you're booking for more than one person:

  1. Each person's information will appear as a separate section
  2. Fill in the details for each person
  3. The contact information (email/phone) is usually shared

Tips for Filling the Form

Name:

  • Use legal names as they appear on ID
  • Avoid nicknames

Date of Birth:

  • Use the dropdown menus for Month, Day, and Year
  • Make sure the date is accurate for eligibility purposes

Email:

  • Use an email you check regularly
  • Confirmation and reminders will be sent here

Phone:

  • Enter 10 digits without dashes or spaces
  • Example: 4165551234

Validation Errors

If you see error messages:

  • Check all required fields - Look for red asterisks (*)
  • Verify email format - Must be valid (e.g., [email protected])
  • Check phone format - Must be 10 digits
  • Verify date of birth - Must be a valid past date

What Happens Next?

After entering all patient information:

  1. Click "Review Appointment"
  2. You'll see a summary of all appointment details
  3. Review and confirm to complete your booking
